Reducing Readmissions: Skills for Success 
The Indiana Hospital Association will host this educational event in an effort to help members reduce avoidable readmissions and prevent financial penalties. Topics will include:

• Project Better Outcomes for Older Adults Through Safe Transitions (Project BOOST);
• Project Re-Engineered Discharge (Project RED);
• Hospital to Home (H2H); and
• State Action on Avoidable Readmissions (STAAR).
